  • Assist individuals with community integration, friendships, family engagement, and community-based resources
  • Plan recreational and social activities promoting social inclusion and emotional well-being
  • Support choice-making, self-advocacy, decision-making, and independent living
  • Support access to assistive technology and adaptive equipment
  • Support work, volunteer, and community engagement opportunities
  • Provide training and support involving rehabilitative and behavioral interventions, fire and safety regulations, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
  • Perform hands-on tasks and provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, communication logs, and activity schedules
  • Procure, store, administer, document, and dispose of medication consistent with MAP requirements, regulations, and company policy
  • Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory policies, procedures, standards, safe work practices, and universal precautions
  • Maintain home safety and perform housekeeping, snow shoveling, and trash removal
  • Provide shift coverage in other homes or programs as assigned
  • Function as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Report to the Disabled Persons Protections Commission as required
  • Assist in developing, implementing, and documenting ISP/PSTPs and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred for employment consideration
  • High school diploma or equivalent required; in some cases, experience may be substituted for academic training
  • Valid driver's license required at time of employment
  • At least 21 years of age
  • Maintained a valid US driver's license for at least six months
  • Adequate insurance and safe driving record if using a personal vehicle
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with daily living and routine living activities
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 90 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Ability to use advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, communication, teaching, and household management techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to communicate effectively in oral and written formats
