About the role
Key responsibilities & impact- Provide direct service in a GLE/SIE residential setting
- Help Persons Served overcome barriers to and prepare for independent living
- Implement Treatment Plan support interventions as assigned by the Team LPHA Clinician
- Administer, document, store, and dispose of medications according to MAP requirements and company policies
- Provide crisis interventions and access on-call backup when needed
- Maintain a safe, comfortable, clean residential environment
- Orient new Persons Served and assist with orientation of new staff
- Collaborate with Integrated Team members to address Persons Served needs
- Perform day-to-day assignments directed by the Program Director
- Apply recovery-oriented, person-centered, harm-reduction, trauma-informed, culturally competent, and strengths-based approaches
- Follow safety, maintenance, Human Rights, operational, licensing, and regulatory requirements
- Teach Persons Served medication management
- Support meal preparation and physical site maintenance
- Encourage social interaction and create a welcoming collaborative environment for residents, families, friends, and community members
- Advocate for Persons Served and serve as a resource for neighbors
- Participate in trainings, supervision, and staff meetings
- Complete required documentation and incident reports
- Contribute to quality improvement goals and objectives
- Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions
- Perform other related duties as assigned
Requirements
What you’ll need- High school diploma or equivalent required
- Associate’s degree in a human service discipline preferred
- One to two years of job-specific experience in human services field preferred
- Driving is required using either a Vinfen van or personal vehicle
- Personal vehicle users must maintain adequate insurance and a safe driving record subject to annual checks
- Valid US driver's license required at employment
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least six months
- Must pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist Persons Served with tasks of daily living
- Ability to remain stationary 50% of the time as needed
- Ability to speak, hear, and communicate with Persons Served, staff, and external representatives
- Specific close, distance, color, peripheral, and depth vision abilities, plus ability to adjust focus
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- Skills and competence to establish supportive trusting relationships with Persons Served, families, and neighbors
- Sensitivity to cultural, religious, ethnic, disability, and gender issues
- Ability to balance competing priorities and make independent judgments and decisions
- Ability to work in a professional and confidential capacity
- Knowledge of personal computer applications and equipment
