About the role

Key responsibilities & impact
  • Provide direct service in a GLE/SIE residential setting
  • Help Persons Served overcome barriers to and prepare for independent living
  • Implement Treatment Plan support interventions as assigned by the Team LPHA Clinician
  • Administer, document, store, and dispose of medications according to MAP policies and procedures
  • Provide crisis interventions and access on-call backup when needed
  • Maintain a safe, comfortable, clean, and compliant residential environment
  • Orient new Persons Served and assist with orientation of new staff
  • Collaborate with Integrated Team members to address Persons Served needs
  • Perform day-to-day assignments directed by the Program Director
  • Use recovery-oriented, person-centered, harm-reduction, trauma-informed, culturally competent, and strengths-based approaches
  • Adhere to safety, maintenance, Human Rights, licensing, and operational policies
  • Teach Persons Served medication management
  • Support meal preparation and physical site maintenance
  • Encourage resident social interaction and a collaborative, welcoming environment
  • Serve as an advocate and resource for Persons Served and neighbors
  • Participate in trainings, supervision, and staff meetings
  • Complete required documentation and incident reports
  • Contribute to quality improvement goals and objectives
  • Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions
  • Perform other related duties as assigned

Requirements

What you’ll need
  • A high school diploma or equivalent is required
  • One to two years of job-specific experience in human services field preferred
  • Driving is required using either a Vinfen van or personal vehicle
  • Personal vehicle users must possess and maintain adequate insurance and a safe driving record
  • A valid US driver's license must be presented at the time of employment
  • Must be at least 21 years of age
  • Must have maintained a valid US driver's license for at least six months
  • Must pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active work
  • Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer and office equipment such as a calculator, copier, and printer
  • Ability to assist Persons Served with tasks of daily living
  • Ability to communicate with Persons Served, staff, and external representatives
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • Strong commitment to Persons Served living and participating in their communities
  • Skills to establish supportive, trusting relationships with Persons Served, families, and neighbors
  • Cultural, religious, ethnic, disability, and gender sensitivity
  • Ability to balance competing priorities and make independent judgments and decisions
  • Ability to work professionally and confidentially
  • Knowledge of personal computer applications and equipment
