About the role

Key responsibilities & impact
  • Develop case management plans to promote participant self-reliance and independence
  • Link families, seniors, and adults to education, employment, budgeting, money management, and supportive services
  • Maintain case management files and document participant progress, plans, follow-up, and placements
  • Evaluate clients’ housing needs and support housing retention
  • Provide crisis management and mediate conflicts
  • Complete recertification agreements, extension requests, discharge paperwork, and participation-agreement compliance activities
  • Assist temporary housing clients with intensive housing searches
  • Create budget and housing stabilization plans
  • Provide resource information and case planning for participants with physical or mental impairments
  • Conduct monthly house meetings and weekly participant meetings
  • Arrange or facilitate transportation when needed
  • Liaise with community service agencies and maintain a list of available services and providers
  • Maintain participant data in HMIS and Credible
  • Communicate daily with the Program Manager
  • Document client concerns and infractions, secure policy exceptions, report workflow problems, and provide emergency shift coverage
  • Assist with orientation and training of new employees
  • Support program development and maintain fire, safety, and health standards
  • Attend staff meetings and assigned training and complete required certifications

Requirements

What you’ll need
  • Bachelor’s Degree in Social Work or a related field preferred
  • Minimum of two years of social work-related education from an accredited academic institution, or three years of experience working with the homeless population or alcohol/drug recovery programs, or a combination thereof
  • Demonstrated ability in record keeping
  • Good written and verbal communication skills
  • Knowledge of issues affecting women, minorities, persons with disabilities, people with a history of substance use, individuals with low to no income, and the unhoused population
  • CPR and First Aid training within 90 days of employment and recertification as necessary
  • Valid California driver’s license
  • Ability to meet the organization’s insurance carrier guidelines
  • Excellent oral and written communication skills
