About the role

Key responsibilities & impact
  • Develop case management plans to increase PSH participants’ self-reliance and independence
  • Link participants to education, employment, budgeting strategies, money management, and supportive services
  • Hold welcome visits and complete strengths and needs assessments
  • Create tenant-driven Individualized Service Plans and Crisis Response Plans; review them at least every 90 days
  • Conduct weekly in-unit check-ins and monthly house meetings
  • Coordinate internal and external care providers as the central hub of each tenant’s care team
  • Maintain participant relationships using Motivational Interviewing, strength-based, and trauma-informed approaches
  • Maintain confidential case management files and document participant progress, follow-up, placements, and outcomes
  • Provide crisis management, conflict mediation, discharge paperwork, and housing stabilization planning
  • Track housing instability, respond to lease violations or eviction notices, and mediate landlord–tenant and tenant–tenant conflicts
  • Coach tenants on budgeting, lease terms, unit upkeep, daily routines, meal planning, and hygiene
  • Assist with move-ins, utilities, household supplies, orientations, lease renewals, and reasonable accommodation requests
  • Maintain participant data in HMIS, Coordinated Entry, and Credible for County reporting
  • Make warm referrals to Nevada County Behavioral Health therapy, case management, and psychiatry
  • Support medical, dental, vision, specialty care, benefits, and CalAIM Enhanced Care Management linkages
  • Assist with SSI/SSDI, Medi-Cal, CalFresh, IHSS, Veterans’ benefits, and representative payee options
  • Maintain relationships with Nevada County Behavioral Health, Nevada County HHSA, landlords, and local providers
  • Communicate daily with the Program Manager and perform program maintenance, reporting, emergency coverage, and related duties

Requirements

What you’ll need
  • Bachelor’s degree in social work or a related field is preferred
  • Minimum of two (2) years’ social work-related education from an accredited academic institution, or three (3) years of experience working with the homeless population or alcohol/drug recovery programs, or a combination thereof
  • Demonstrated ability in record keeping
  • Proficient written and verbal communication skills
  • Knowledge of problems unique to women, minorities, persons with disabilities, persons with a history of substance use, individuals with low to no income, and the general unhoused population
  • CPR and First Aid training within ninety (90) days of employment and recertification as necessary
  • Valid driver’s license, reliable transportation, and ability to meet organization’s insurance carrier guidelines
