About the role
Key responsibilities & impact- Provide clients with comprehensive assessments and ensure a high-quality client experience
- Develop comprehensive care plans in consultation with clients and caregivers
- Support clients and caregivers in managing care budgets and required resources
- Interpret and complete standardized assessment tools, including interRAI CHA, Supports Intensity Scale, and Application for Developmental Services
- Complete client assessments using conversation-based methodology
- Build therapeutic rapport with clients
- Identify and prioritize environmental risks affecting client health and safety
- Facilitate access to client-identified services and appropriate community resources
- Make referrals to community and health resources
- Consult with care partners on service plans, client needs, and interventions
- Monitor client progress and adjust care plans as required
- Review care plan expenditures and support clients in managing budgets
- Collect and report relevant program outcome data to the Manager and funder
- Maintain privacy, confidentiality, and information-protection standards
- Represent providers, stakeholders, community agencies, and service providers professionally to clients
- Mediate and problem-solve with multiple parties
- Promote VON goals, values, and integrated community care
- Promote a safe and healthy workplace and comply with occupational health and safety requirements
- Collaborate with staff and regional partners in team-based service delivery
- Work from a home office and in the community 2–3 times per week to complete assessments
- Travel to the Trenton office and Kingston area as needed
Requirements
What you’ll need- University Degree or College Diploma in Health or Social Services field
- Minimum of 3 years related experience in a home and community care organization or social services program
- Experience in case management and building external and community relationships
- Experience completing validated common assessment tools and maintaining competency
- Broad working knowledge of community resources and programs within the defined catchment area
- Knowledge and skills in comprehensive assessment
- Computer proficiency in Windows OS and Microsoft Office programs
- Analytical skills and ability to understand and interpret client assessment data
- Ability to manage multiple tasks, prioritize, meet deadlines, and work under pressure
- Knowledge of social and developmental service sectors may be required
- Skills in leadership and team building
- Ability to educate clients and families on available program resources
- Strong time management and organizational skills
- Ability to work independently and as part of a team
- Ability to accept and facilitate change
- Ability to maintain confidentiality
- Available for weekend assessment coverage
- Access to a personal vehicle for business use
- Current driver’s license and proof of vehicle insurance
- Current and original satisfactory Criminal Records Check
- Vulnerable Sector Search and/or Child Abuse Registry Check may be required
- Ability to use required Personal Protective Equipment (PPE)
