About the role

Key responsibilities & impact
  • Manage pediatric, palliative, and/or acute medically complex clients with COPD or CHF
  • Direct Personal Support Workers/Health Care Technicians in providing client care
  • Provide remote assessment and management of care services in clients’ homes
  • Provide clinical direction and assign tasks within PSW/HCT practice statements
  • Maintain accurate digital and paper clinical records
  • Maintain patient assignments within the virtual ward
  • Collaborate with team members to meet patient care needs
  • Participate in daily huddles and respond to emails and phone calls
  • Communicate with outside service provider agencies and Case Managers
  • Participate as a member of the multidisciplinary healthcare team
  • Provide clinical leadership, direction, and mentorship
  • Facilitate a collaborative team-based environment
  • Establish and maintain relationships with local healthcare providers
  • Participate in CNO Reflective Practice
  • Uphold VON standards and eHomecare processes
  • Participate in internal projects and continuous improvement initiatives
  • Demonstrate professionalism and adaptability in a fast-paced virtual environment

Requirements

What you’ll need
  • Registered and in good standing with the College of Nurses of Ontario
  • CNA Certificate in Palliative Care, or completion of both “Fundamentals of Palliative Care” and “Comprehensive Advanced Palliative Care Education (CAPCE)”
  • Pediatric DRNs must have extensive hands-on experience in Pediatric Care
  • Recent palliative, pediatric, and/or acute medical experience with COPD and CHF patients
  • Demonstrated advanced assessment skills
  • Experience in gerontology and/or medical-surgical nursing
  • Community-based health care experience is an asset
  • Competency in observation, independent assessment, health education and promotion, and COPD/CHF clinical pathways
  • Digital and electronic health experience, EMR and mobile technology
  • Proficient nursing skills and ability to interpret electronic technical information related to client outcomes
  • Critical thinking, problem-solving and multitasking skills
  • Ability to work in a multidisciplinary virtual team and independently
  • High-speed internet, computer skills, and ability to work from home in a private office area
  • Eligible to successfully obtain a Government of Canada security screening certificate
  • Valid CPR C / First Aid
  • Personal vehicle, current driver’s license, and proof of vehicle insurance
  • Satisfactory Criminal Records Check required upon starting employment
  • Vulnerable Sector Search and/or Child Abuse Registry Check may be required
  • Ability to speak French is an asset in French Designated areas
  • Ability to wear Personal Protective Equipment (PPE)
  • Must meet the physical demands of lifting, carrying, bending, reaching, kneeling, walking, sitting, standing, climbing stairs, and fine hand movements
