About the role
Key responsibilities & impact- Serve as the client’s primary point of contact and coordinate with doctors, specialists, pharmacists, social services providers, caregivers, family supports, and other care-team members
- Manage client cases and coordinate health care benefits
- Provide education and facilitate timely, cost-effective access to care
- Promote wellness, recovery, independence, resilience, and member empowerment
- Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community services
- Oversee development of client care plans and goals
- Provide office-based, telehealth, or field-based services where members reside, seek care, or can most easily access services
- Connect clients with needed social services and supports
- Advocate for clients with health care professionals
- Use Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
- Conduct outreach and engagement to facilitate linkage to the ECM program and log activities in the CRM system
- Evaluate client progress and update SMART goals
- Provide mental health promotion
- Arrange transportation, including ACCESS
- Complete documentation and outcome measures within care-plan timeframes
- Maintain current patient health records in the EMR and other business systems
- Complete monthly reporting for program compliance
- Attend assigned training
Requirements
What you’ll need- Active, unrestricted LCSW, LMFT, or LPCC license in California required
- 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Willing and able to work Monday-Friday, 8:30am-5:00pm, with flexibility for potential evenings and weekends
- Willing and able to work both in the field and remotely
- Working knowledge of government and community resources related to social determinants of health
- Excellent oral and written communication skills
- Positive interpersonal skills
- Clean driving record, valid driver's license, and reliable transportation
- General computer skills and working knowledge of Google Workspace, MS Office, and the internet
- Bilingual English/Spanish highly preferred
- Must reside within 20 miles of the assigned Humboldt County territory
- Willing and able to travel throughout the assigned territory up to 5 days per week
- Legally authorized to work in the United States
- Must complete background screening before employment
- Must be vaccinated against influenza for patient-, client-, or customer-facing work, subject to accommodation review
