About the role
Key responsibilities & impact- Serve as the client’s primary point of contact and coordinate with doctors, specialists, pharmacists, social service providers, caregivers, family support persons, and other care-team members
- Manage client cases and coordinate health care benefits
- Provide education and facilitate timely, cost-effective access to care
- Promote wellness, recovery, independence, resilience, and member empowerment
- Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services
- Oversee development of client care plans and goal setting
- Offer office-based, telehealth, or field-based services where members reside, seek care, or can access services most easily
- Connect clients with needed social services and supports
- Advocate for clients with health care professionals
- Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
- Conduct outreach and engagement activities and log activity in the CRM system
- Evaluate client progress and update SMART goals
- Provide mental health promotion
- Arrange transportation, such as ACCESS
- Complete documentation and outcome measures within care-plan timeframes
- Maintain current patient health records in the EMR and other business systems
- Complete monthly reporting for program compliance
- Attend assigned training
- Travel throughout the Los Angeles area up to 5 days per week and work in hybrid in-field and remote settings
Requirements
What you’ll need- 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, in the field and remotely, with flexibility for potential evenings and weekends
- Working knowledge of government and community resources related to social determinants of health
- Clean driving record
- Valid driver's license
- Reliable transportation
- Excellent oral and written communication skills
- Positive interpersonal skills
- General computer skills
- Working knowledge of Google Workspace, MS Office, and the internet
- Bilingual English/Spanish highly preferred
- Must be legally authorized to work in the United States
- Must comply with influenza vaccination requirements for patient-, client-, or customer-facing roles
- Background check, potentially including drug test or other health screenings, required prior to employment
