About the role

Key responsibilities & impact
  • Serve as the client's primary point of contact
  • Coordinate with doctors, specialists, pharmacists, social services providers, caregivers, family support persons, and other Care Team members
  • Manage client cases and coordinate health care benefits
  • Provide education and facilitate timely, cost-effective access to care
  • Promote wellness, recovery, independence, resilience, and member empowerment
  • Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services
  • Oversee development of client care plans and goal setting
  • Offer office-based, telehealth, or field-based services where members reside, seek care, or can most easily access services
  • Connect clients to social services and supports
  • Advocate for clients with health care professionals
  • Use Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
  • Conduct outreach and engagement activities and log activity in the CRM system
  • Evaluate client progress and update SMART goals
  • Provide mental health promotion
  • Arrange transportation such as ACCESS
  • Complete documentation and outcome measures within care-plan timeframes
  • Maintain current patient health records in the EMR and other business systems
  • Complete monthly reporting for program compliance
  • Attend assigned training

Requirements

What you’ll need
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends
  • Willing and able to work both in the field and remotely
  • Working knowledge of government and community resources related to social determinants of health
  • Clean driving record
  • Valid driver's license
  • Reliable transportation
  • Excellent oral and written communication skills
  • Positive interpersonal skills
  • General computer skills
  • Working knowledge of Google Workspace, MS Office, and the internet
  • Must reside within a 20-mile radius of the assigned territory
  • Ability to travel throughout Orange County up to 5 days per week
  • Vaccination against influenza for patient-, client-, or customer-facing roles
  • Ability to complete identity and work eligibility verification using E-Verify upon hire
  • Bilingual English/Spanish highly preferred
