About the role

Key responsibilities & impact
  • Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services
  • Oversee development of client care plans and goal setting
  • Offer office-based, telehealth, or field-based services where members reside, seek care, or can most easily access services
  • Connect clients with needed social services and supports
  • Advocate for clients with healthcare professionals
  • Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
  • Conduct outreach and engagement to facilitate linkage to the Enhanced Care Management program and log activities in the CRM system
  • Evaluate client progress and update SMART goals
  • Provide mental health promotion
  • Arrange transportation
  • Complete documentation and outcome measures within care-plan timeframes
  • Maintain current patient health records in the EMR and other business systems
  • Complete monthly reporting for program compliance
  • Attend assigned training
  • Coordinate person-centered care with healthcare providers, specialists, pharmacists, social service agencies, caregivers, family supports, and other care-team stakeholders
  • Coordinate healthcare benefits and facilitate timely, cost-effective access to care
  • Promote wellness, recovery, independence, resilience, and member empowerment

Requirements

What you’ll need
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Must be willing and able to work Monday-Friday, 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends
  • Working knowledge of government and community resources related to social determinants of health
  • Clean driving record
  • Valid driver's license
  • Reliable transportation
  • Excellent oral and written communication skills
  • Positive interpersonal skills
  • General computer skills
  • Working knowledge of Google Workspace, MS Office, and the internet
  • Bilingual English/Spanish preferred
  • Must be legally authorized to work in the United States
  • Must complete background screening, which may include a drug test or other health screenings
  • Patient-, client-, or customer-facing employees must be vaccinated against influenza, subject to applicable accommodation review
