About the role

Key responsibilities & impact
  • Serve as the client’s primary point of contact under the direction of the ECM Clinical Manager
  • Coordinate with doctors, specialists, pharmacists, social services providers, caregivers, family support persons, and other care-team members
  • Manage client cases and coordinate health care benefits
  • Provide education and facilitate timely, cost-effective access to care
  • Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community services
  • Oversee development of client care plans and goals
  • Offer office-based, telehealth, and field-based services where members reside or seek care
  • Connect clients with social services and community supports
  • Advocate for clients with health care professionals
  • Apply Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
  • Conduct outreach and engagement and log activity in the CRM system
  • Evaluate client progress and update SMART goals
  • Provide mental health promotion
  • Arrange transportation, including ACCESS
  • Complete documentation and outcome measures within care-plan timeframes
  • Maintain current patient records in the EMR and other business systems
  • Complete monthly compliance reporting
  • Attend assigned training

Requirements

What you’ll need
  • Active, unrestricted California Licensed Vocational Nurse (LVN) license required
  • Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Working knowledge of government and community resources related to social determinants of health
  • Excellent oral and written communication skills
  • Positive interpersonal skills required
  • Clean driving record, valid driver's license, and reliable transportation
  • General computer skills and working knowledge of Google Workspace, MS Office, and the internet
  • Bilingual (English/Spanish), highly preferred
  • Must reside within 20 miles of the assigned Humboldt County territory
  • Must be legally authorized to work in the United States
  • Must pass required background screening
  • Patient-facing employees must be vaccinated against influenza, subject to accommodation consideration
