About the role

Key responsibilities & impact
  • Own and execute the full onboarding workflow for an assigned portfolio of physician partners
  • Manage data collection, agreements, attestations, system setup, access provisioning, and partner education
  • Lead activation readiness, monitor milestones, resolve blockers, and ensure compliant handoffs to Service Activation
  • Lead executive-level onboarding engagements, kick-off meetings, partner assessments, SME sessions, training webinars, and orientation events
  • Deliver education on Annual Wellness Visits, HCC coding, and ACO foundational program requirements
  • Own partner relationships throughout onboarding and drive partner performance, satisfaction, and engagement
  • Execute onboarding and offboarding workflows in the AI Platform while maintaining data accuracy and compliance
  • Identify process-improvement opportunities and recommend automation and technology-roadmap enhancements
  • Develop and maintain onboarding tools, templates, project plans, training materials, and handoff documentation
  • Support partner offboarding, including exit timelines, data retrieval, system deprovisioning, and compliance closure
  • Coordinate with Growth, Partner Success, Service Activation, Analytics, Compliance, and Technology/Product
  • Provide portfolio updates, partner pipeline summaries, and activation-readiness reporting to senior stakeholders
  • Analyze partner data and onboarding outcomes to identify trends, risks, and improvement opportunities
  • Continuously improve onboarding best practices, SOPs, and partner-facing education materials
  • Stay current on ACO updates, value-based care developments, operational changes, and Business Transformation priorities

Requirements

What you’ll need
  • Bachelor’s degree required
  • MBA, MPH, or MHA strongly preferred
  • 5+ years of relevant healthcare industry experience in operations, partner/client onboarding, or program management
  • Healthcare consulting or operations management background preferred
  • Direct experience with ACO programs, value-based care operations, or physician practice onboarding strongly preferred
  • Experience working cross-functionally across Growth, Partner Success, Service Activation, Analytics, Compliance, and Technology/Product
  • Strong working knowledge of MSSP requirements, CMS compliance standards, HCC coding, Annual Wellness Visits, and ACO program fundamentals
  • Experience building relationships with physician partners and practice administrators
  • Strong verbal, written, and presentation skills
  • Comfortable working with partner data and lifecycle metrics
  • Proficiency in Salesforce, AI-enabled onboarding platforms, Monday.com, Jira, Smartsheet, and Microsoft Office Suite
  • PMP or equivalent project/program management credential is a plus; no specific certifications required
