About the role
Key responsibilities & impact- Provide specialized, harm-reduction-focused support to adults with HIV, adherence-related issues, or transferring care
- Meet with patients face to face, virtually, and by phone to provide education, counseling, support, and referrals
- Work as part of a multidisciplinary team with medical providers, community health workers, behavioral health staff, and other care team members
- Identify patients needing specialty adherence care and encourage appropriate referrals
- Systematically engage patients who are out of care and refer them to community agencies as needed
- Guide patients through Whitman-Walker and external services as a patient representative and advocate
- Make regular patient contact and develop individualized care plans supporting treatment adherence and long-term retention
- Identify and address barriers to retention and gaps in care
- Follow up with hospitalized patients, ER visitors, missed clinic visits, missed medication refills, or potentially non-adherent patients
- Coordinate with clinicians, case managers, behavioral health providers, and other healthcare providers
- Maintain accurate records in EMR and other data systems
- Prepare narrative, statistical, weekly, monthly, and annual reports
- Maintain a current resource list of community service organizations
- Participate in quality improvement activities, adherence training, and ongoing education
- Follow Whitman-Walker policies and procedures, including HIPAA requirements
- Perform other duties as assigned, including emergency-response tasks
Requirements
What you’ll need- Minimum two or more years of paid work experience providing direct assistance to a diverse population in public health, healthcare, human services, or other community settings
- Bachelor's Degree in psychology, social work, public health, or related field, or at least four years of direct experience
- Experience with HIV and/or STD CTS and HIV outreach, linkage, and retention services preferred
- Experience working with an ethnically, culturally, and racially diverse work staff preferred
- Ability to work harmoniously with diverse groups of individuals required
- Knowledge of HIV/AIDS-related illnesses and substance abuse, including current treatment and adherence strategies
- Knowledge of chronic health conditions, including diabetes, hypertension, and heart disease
- Knowledge of patient needs assessment, risk-reduction and referral plans, community resources, and documentation
- Knowledge of Ryan White HIV/AIDS Program and Medicaid
- Excellent oral and written communication skills
- Knowledge of healthcare information systems and ability to adapt to electronic medical records and population health platforms
- Analytical skills for accurate, concise, and comprehensive reports
- Ability to maintain accurate, timely, and confidential records
- Flexibility to work afternoon, evening, and weekend shifts
- Commitment to health equity
- Ability to work independently with minimal supervision
- Ability to lift no more than 20 lbs. infrequently
- Written and verbal competency
