About the role

Key responsibilities & impact

Oversee incoming Preferred Communities referrals and conduct comprehensive intake assessments and client enrollments
Provide Intensive Case Management for individuals and families
Develop and implement individualized goal plans supporting long-term self-sufficiency
Maintain proactive communication with clients, monitor progress, address emerging needs, and provide ongoing support
Guide clients through healthcare, social service, and community systems
Maintain timely documentation and detailed case notes in accordance with program and compliance standards
Collaborate with the Programs Manager and program staff across departments
Partner with the Volunteer Coordinator to engage and leverage volunteers and community groups
Connect clients with wrap-around community services and advocacy networks
Maintain compliance across Preferred Communities programming and service delivery
Ensure timely and accurate data collection and outcome tracking
Participate in meetings, staff development, trainings, community activities, and organizational events
Submit monthly, semi-annual, and annual reports to the program manager
Maintain accurate service files and records
Ensure continuity of services and reporting during staff transitions and in the absence of a Programs Manager
Adhere to World Relief policies and procedures
Travel between High Point and Winston-Salem office locations
Perform other assigned tasks and projects

Requirements

What you’ll need

Mature and personal Christian faith
Commitment to World Relief’s mission, vision, and values
Desire to serve and empower the Church to impact vulnerable communities
Ability to affirm World Relief’s Core Beliefs and Statement of Faith
Acknowledgment of World Relief’s Christian Identity and National Evangelicals For the Health of The Nation document
Valid driver’s license
Access to a reliable vehicle
Ability to pass MVR standards
Experience in case management, health education, or related fields required
Understanding of challenges faced by refugees and immigrants, including trauma-informed care and cultural sensitivity
Strong organization and ability to manage multiple cases and prioritize tasks
Excellent verbal and written communication skills
Strong interpersonal skills, including empathy, active listening, and conflict resolution
Fluency in English required
Bilingual ability in Arabic, Swahili, Spanish, French, Kareni, Burmese, Pashto, or Dari
