Community Health Worker

• Serve as a community-based, patient-facing resource, building relationships with patients and caregivers through direct, in-person re-engagement in assigned geographic areas
• Provide hands-on support with program re-enrollment, onboarding, and mobile app setup and utilization
• Perform telephonic outreach to caregivers to re-engage children in care and support onboarding
• Educate patients and caregivers on Imagine Pediatrics services and community resources
• Maintain knowledge of local community resources and connect families to appropriate services
• Conduct community events and facility rounding
• Partner with Behavioral Health teams and local behavioral health providers on discharge planning and community connections
• Identify barriers to engagement and access to care with caregivers, families, and facility staff
• Collaborate with the Patient Engagement Team, Outreach Specialists, and Registered Nurses
• Support non-clinical care coordination, including primary care connections, pharmacy access, discharge follow-up, SDOH needs, and removing care barriers
• Support outbound engagement calls for enrollment, follow-up, and patient engagement metrics
• Perform other duties as assigned
