Program Coordinator

• Support the Program Director in the day-to-day administration and operation of the NY Connects Brooklyn program.
• Conduct routine reviews of client records, staff notes, assessments, referrals, and other program documentation.
• Ensure documentation is accurate, complete, timely, and consistent with program requirements.
• Identify deficiencies, communicate required corrections, maintain QA tracking systems, and follow up on corrective actions.
• Monitor staff adherence to policies, procedures, documentation standards, and contractual requirements.
• Assist with internal audits, funder monitoring, site visits, and compliance reviews.
• Monitor contractual deliverables, performance metrics, productivity, and established targets.
• Maintain databases, spreadsheets, dashboards, referral tracking systems, and performance-tracking systems.
• Review and correct data discrepancies and compile data for internal and external reporting.
• Analyze QA findings and performance data to identify trends and recommend process improvements.
• Coordinate daily administrative and operational activities, schedules, staffing coverage, assignments, meetings, files, onboarding, and training records.
• Monitor referral activity, follow-up activities, response timeliness, and service-delivery trends.
• Provide staff technical assistance regarding documentation, QA standards, program procedures, and workflow.
• Prepare QA, performance, operational, and compliance reports and provide timely updates to the Program Director.
• Report to the Program Director, NY Connects.
