About the role

Key responsibilities & impact

Assess and screen patients' daily living and work-related skills
Develop therapeutic retraining programs with measurable objectives
Administer and interpret diagnostic and prognostic functional tests in patients' homes or other settings
Confer with physicians and healthcare team members to develop and revise Plans of Care
Provide therapeutic treatment and instruction to improve or restore strength, coordination, range of motion and function
Review and expand retraining programs according to patient behavioral goals and rehabilitation guidelines
Develop vocationally oriented plans with vocational team members
Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients and families
Coordinate care and discharge planning

Recommend or design adaptive equipment
Train staff and family members in retraining programs and daily living skills
Supervise, instruct and evaluate Occupational Therapy Assistants and Home Health Aides
Document patient care observations, interventions and evaluations on the day services are rendered
Document and implement interim physician orders
Submit evaluations, treatment plans and discharge summaries to supervisors and care management staff

Requirements

What you’ll need

Current and unrestricted OT licensure
Current CPR certification
Valid driver's license
Auto insurance
Reliable transportation
Must maintain personal vehicle liability insurance in accordance with residing-state minimum limits or specified higher limits
Minimum six months of occupational therapy experience (preferred)
Home Health experience (preferred)
Ability to provide patient-facing care and participate in Humana's TB screening program
