About the role
Key responsibilities & impact- Act in accordance with the organization’s values, maintaining a results-oriented approach to patient care and respecting each patient’s individuality
- Record all relevant and essential information in the patient’s medical record as part of the care process, keeping it up to date
- Represent the organization by providing care based on scientific evidence and best practices
- Provide comprehensive, empathetic, and compassionate patient care, building strong relationships and considering patients’ biopsychosocial needs
- Ensure continuity of medical care through the implementation of the established care plan
- Manage the appointment schedule appropriately, fulfilling the contracted working hours and required number of consultations
Requirements
What you’ll need- Medical degree with residency training or postgraduate qualification in Geriatrics and/or Family and Community Medicine
- Active registration with the Regional Council of Medicine (CRM)
- Previous experience in home-based care or community health will be considered an advantage
- Passion for promoting patient self-care and autonomy through a person-centered approach
- Strong communication and empathy skills
- Ability to work collaboratively within a multidisciplinary team
- Proactive approach to identifying solutions that promote patients’ quality of life and functional independence
