About the role
Key responsibilities & impact- Cover day room staff absences at the PACE center
- Cover escorts for participants to external appointments
- Assist PACE Center participants with personal care, restorative rehabilitation, meals, group and individual activities
- Accompany participants on transportation as needed to ensure safety
- Assist supported housing participants with morning care as assigned
- Contribute ideas for the activities program and suggestions to improve daily operations
- Contribute to participant care planning and assessment through reports to the Activities Coordinator, PACE Center Nurses, and Rehabilitation staff
- Float to other sites to support patient care needs across the program
Requirements
What you’ll need- High School graduate or GED
- Certification of Nursing Assistant or Home Health Aide
- Experience working with older adults or frail populations preferred
- Ability to provide care and demonstrate respect for frail elders
- Creativity and confidence in leading group activities
- Good communication skills
- Able to work effectively with fellow PCA’s and the Interdisciplinary Team
- Flexibility, initiative, and organizational skills
- Bilingual a plus
- Must complete a background check before beginning employment
