About the role
Key responsibilities & impact- Provide nursing visits for patients as delegated by the Case Manager RN
- Provide continuity of care for patients
- Provide nursing care in patients’ places of residence as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies
- Keep the Case Manager RN informed about patient progress, recommended actions, physician orders, and plans for next visits
- Participate as a member of the interdisciplinary team
- Provide on-call support as assigned
- Participate voluntarily in the per diem on-call rotation
- Perform emergency tasks such as patient resuscitation in the home
- Maintain awareness of personal safety in varied and unpredictable situations
Requirements
What you’ll need- Associate’s degree required
- Current Washington State Registered Nurse license
- Current BLS certification required
- Valid Washington State driver’s license required
- Proof of insurance on automobile used for work required
- Driving record in compliance with OMC standards required
- Minimum of one-year recent experience in home health, acute or long-term care preferred
- Must have computer internet connectivity
- Must have reliable automobile transportation to travel to patient residences
- Ability to apply nursing standards and principles
- Problem-solving skills and ability to make independent decisions in complex and diversified work situations
- Keen awareness of personal safety
- Ability to perform strenuous exertion under emergency conditions

Comp & perks- Per diem premium of 15% on base plus steps, in lieu of benefits
- Weekend premium: $4.00/hour
- Lead premium: $3.25/hour
- Preceptor premium: $1.00/hour
- Certification premium: $2.00/hour
- Weekend Coordinator Pay: 15% on regular pay
