About the role
Key responsibilities & impact- Support the Director of Advanced Practice Providers and Nurse Practitioners in delivering high-quality healthcare services
- Review and administer care plans for RPM, CHI, and CCM services
- Conduct telehealth visits and enhance care plans for existing patients
- Meet productivity benchmarks for billable time
- Review and analyze patient data collected through RPM devices and intervene timely on health concerns
- Educate patients on RPM technology and troubleshoot device-related issues
- Assist NPs in interpreting complex RPM data and implementing care strategies
- Develop and oversee comprehensive CCM care plans with NPs
- Follow up with patients regarding medication adherence, lifestyle changes, education, and compliance
- Participate in CHI programs targeting underserved populations and address social determinants of health
- Conduct comprehensive health assessments and support acute and chronic care needs
- Deliver telehealth consultations, triage patient inquiries, manage follow-ups, and address immediate concerns
- Coordinate with Medical Assistants, Care Navigators, NPs, and local health organizations
- Create patient care plans for all new enrollments
- Work with complex cases and multi-diagnosis patients
- Conduct daily chart reviews
- Schedule escalation visits for patients with changed health statuses
- Participate in training new MA hires
- Facilitate warm handoffs and act as a liaison among interdisciplinary team members
- Maintain accurate records and ensure HIPAA and telehealth regulatory compliance
- Audit patient documentation
- Track and report RPM, CCM, and CHI metrics, including engagement rates and program outcomes
- Improve clinical processes based on performance data and patient feedback
- Support quality assurance initiatives and organizational benchmarks
Requirements
What you’ll need- Active and unrestricted RN license in the State of New York is required
- Bachelor’s or Associate’s degree in Nursing (BSN preferred)
- Experience in telehealth, RPM, CCM, or CHI services is highly desirable
- Strong communication, organizational, and problem-solving skills
- Demonstrated ability to work collaboratively in a team-oriented, patient-focused environment
- Bilingual fluency in Spanish is required
- Monday-Friday, 9 am-5 pm schedule
- Ability to work in a hybrid arrangement: 3 days in office and 2 days from home
