About the role
Key responsibilities & impact- Provide clinical care for patients
- Complete comprehensive patient assessments
- Determine diagnosis and treatment of illness
- Order and interpret medical diagnostics in tandem with attending physician
- Prescribe medications, treatments, therapy and medical equipment
- Conduct patient education for chronic illness or wellness care
- Refer patients for specialty care when determined
- Consult with physicians when needs exceed scope of practice or diagnosis/treatment plans are unclear
- Promote communication within interdisciplinary health care teams
- Act as a resource to team members
- Work with primary care team on program development
- Act as liaison with outside agencies as needed
- Consult and round on patients with hospital staff when designated by the specialty
- Work independently or as part of a treatment team in inpatient and outpatient settings
- Serve as educator, case manager and consultant to staff and multidisciplinary health care teams
- Perform other duties as assigned
Requirements
What you’ll need- Master's degree - Completion of accredited Nurse Practitioner program
- Licensed Registered Nurse - MN Board of Nursing required upon hire
- Certified Nurse Practitioner - American Nurse Credentialing Center (ANCC) certification within specialty required upon hire
- Certified Nurse Practitioner - MN Board of Nursing required upon hire
- ACLS-BLS Tier 2 - Multisource required within 180 Days
- Drug Enforcement Administration required upon hire
- Valid Driver’s License required upon hire
- MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment for those working in Complex Care for Seniors, Community Palliative Care and Hospice
- Minimum 2 years experience in palliative care, internal medicine, family practice, hospital medicine, or oncology
- Medium Work: lifting weight up to 32 lbs. occasionally and up to 25 lbs. frequently
