About the role
Key responsibilities & impact- Provide on-site clinical social work care across Galion Dialysis, Mid Ohio Dialysis, and Akron Renal Dialysis
- Support patients' psychosocial health and wellbeing through interventions, mandatory assessments, screenings, and education
- Manage a caseload of approximately 100–125 patients across three clinics
- Build meaningful, long-term relationships with patients and their families
- Support and advocate for dialysis patients
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease
- Work independently as part of each patient's core care team
- Collaborate with and participate in social worker meetings, typically at most once per quarter
Requirements
What you’ll need- Licensed in Ohio
- Meets all state required regulations to practice in the dialysis setting
- Must hold an LSW licensure within the state you are applying
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook
- Functional proficiency with DaVita-specific applications within 60 days
