About the role
Key responsibilities & impact- Provide psychosocial health and wellbeing support to dialysis patients
- Provide interventions as needed
- Complete mandatory assessments, screenings, and education
- Manage a caseload of approximately 100–125 patients across three clinics
- Build meaningful, long-term relationships with patients and their families
- Support and advocate for dialysis patients
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease
- Work as a vital member of each patient’s core care team
- Cover Akron Renal Dialysis, Mid Ohio Dialysis, and Galion Dialysis
- Attend social worker meetings, typically at most once per quarter
Requirements
What you’ll need- Licensed Social Worker licensed in Ohio
- Meets all state required regulations to practice in the dialysis setting
- Must hold an LSW licensure within the state of application
- Master's degree in Social Work (MSW) required, with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook
- Functional proficiency with DaVita-specific applications within 60 days
- Empathy and good personal skills
- Ability to work independently
- Schedule availability of 32–40 hours per week, Monday–Friday, 8:00am–4:30pm, with some flexibility depending on patient needs
