About the role
Key responsibilities & impact- Conduct in-home visits to gather health information, monitor wellness, and report changes or concerns to the RN
- Train direct support staff on medical procedures, health-related tasks, and wellness practices
- Collaborate with the RN and management team when health needs change or additional support is needed
- Support new enrollments involving individuals with significant medical needs
- Review provider referrals to help coordinate care and support informed health decisions
- Participate in team meetings and work groups to strengthen communication, collaboration, and quality of care
- Provide health support and wellness education under the direction of a Registered Nurse (RN)
- Build relationships through in-home visits, phone consultations, and collaboration with team members
Requirements
What you’ll need- Current Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing
- Experience in home or community-based care is preferred
- Ability to explain health information and procedures clearly to staff and team members
- Ability to manage time, balance multiple priorities, and respond to changing needs
- Compassionate, team-oriented mindset and commitment to supporting the well-being of others
- Openness to learning, professional development, and continuous improvement
- Valid driver's license in good standing
- Reliable transportation to get to and from work
- Ability to pass a background check with the MN DHS
- Must be 18 or older
- Must be a U.S. citizen or authorized to work in the U.S.
