About the role
Key responsibilities & impact- Identify and collect referral information supporting patient eligibility for care
- Make “live” calls with the Area Vice President of Sales to foster relationships with key account staff
- Call on assigned key accounts with the Account Executive and according to the Quarterly Business Review plan
- Make calls and collaborate on referrals to service referral opportunities
- Communicate with patients’ families regarding services offered
- Interact with hospital staff across departments
- Consolidate and review patient records during the discharge process
- Collaborate with hospital staff to position patients for appropriate care
- Explain care options to patients and families
- Collaborate with healthcare facilities on discharge planning
- Coordinate with healthcare facility staff to ensure smooth transitions to the home environment
- Collaborate with Intake on insurance eligibility and coverage
- Coordinate communication among patients, physicians, case managers, and other medical staff
- Develop positive community relationships
- Perform other assigned duties and projects
- Report to the Area Vice President of Sales
Requirements
What you’ll need- High School Diploma/GED
- 3+ years’ practical experience
- LPN/RN License is preferred
- Bachelor’s Degree is preferred
- Experience in Case Management in a clinical setting is preferred
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with the laws of the State
