About the role
Key responsibilities & impact- Provide rehabilitative services to residents under specific medical orders as part of a coordinated team
- Plan initial and subsequent treatment programs based on clinical findings
- Participate in clinical conferences to share and coordinate patient plans of care
- Provide individualized, one-on-one treatment sessions with a functional approach
- Collaborate with residents, families, and the interdisciplinary team to support meaningful outcomes
- Provide therapy throughout the senior living community, including residents’ apartments
- Support residents in maintaining independence and quality of life
- Work with the Rehabilitation Director and established rehabilitation team
Requirements
What you’ll need- Degree in Physical Therapy
- Current Physical Therapy license in the state of Illinois, or ability to obtain prior to beginning employment
- Excellent interpersonal, communication, clinical, and collaborative skills
- Commitment to providing individualized, resident-centered care
- Ability to work a full-time salaried schedule of 40 guaranteed hours per week
- Ability to work Monday–Friday with no weekends
- Ability to treat approximately 6–7 patients per day
- 80% productivity expectation
