About the role
Key responsibilities & impact- Provide direct behavioral health care using an evidence-based, patient-centric approach
- Conduct virtual visits and manage a caseload consistent with full-time or part-time expectations
- Document patient encounters, coordinate follow-up care, and review comprehensive care plans
- Address behavioral, emotional, and psychological barriers to sustained lifestyle and metabolic change
- Collaborate with registered dietitians, nurse practitioners, physicians, and other care team members
- Use data and feedback to improve care delivery, patient outcomes, and team coordination
- Maintain high standards of integrity, ethics, and confidentiality
Requirements
What you’ll need- Active, independent, and unrestricted clinical license in the U.S. (LCSW / LPC / LMFT / or equivalent)
- Multi-state licensure is a strong plus
- Experience with telehealth
- Experience supporting patients with behavior change, chronic disease, or related clinical needs
- Ability and enthusiasm to work in a team-based telehealth/virtual care environment
- Strong clinical judgment, empathy, and excellent communication skills
- Commitment to evidence-based practice and long-term behavioral change
- Ability to integrate with a multidisciplinary care team and leverage technology to support care delivery
- Must be authorized to work and physically based in the U.S.
