About the role
Key responsibilities & impact- Deliver skilled nursing care in patients’ homes
- Create individualized care plans
- Administer medications
- Educate patients and families
- Support patient recovery and independence
- Provide meaningful one-to-one patient care
- Document care and coordinate services using Epic EHR
- Work three 12-hour shifts from Thursday through Saturday
- Build relationships with patients and observe the direct impact of care
- Collaborate with care teams in a home health, hospice, and palliative care organization
Requirements
What you’ll need- Graduate of an approved Nursing education program
- Current Registered Nurse licensure in the State of Michigan
- Minimum of one (1) year professional registered nursing experience
- Strong computer and technology skills required
- Current driver's license and reliable transportation
- Medical Surgical experience preferred
- Home health or Hospice experience is a plus
- BSN preferred
