About the role
Key responsibilities & impact- Deliver patient care services through a remote technology platform
- Independently provide holistic, evidence-based care for patients aged 18 months and above
- Assess, diagnose, treat, and manage health problems
- Provide health counseling and disposition planning
- Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices
- Evaluate primary care, acute, chronically ill, and transitional care patients
- Provide contraceptive care counseling and medication management
- Educate patients on health maintenance and respond to patient care inquiries
- Document all patient care in an EHR according to MinuteClinic policies and procedures
- Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home
- Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members
- Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice
Requirements
What you’ll need- Currently licensed in Tennessee
- Minimum of two years of medically-relevant experience or equivalent
- Completion of a Master’s Degree level Family Nurse Practitioner program
- Current National Board Certification
- State of Employment license to practice in the Advanced Practice Nurse role
- Effective verbal, written, and electronic communication skills
- Outstanding organizational skills and ability to multi-task
- Initiative, problem solving ability, adaptability and flexibility
- Ability to work without direct supervision and practice autonomously
- Proficiency with information management and technology
- Capacity to collaborate with professional colleagues
- Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required
- Broadband connectivity and a quiet setting with a neutral background for remote care delivery
- Ability to uphold patient privacy per CVSH guidelines
- Ability to hardwire into the internet connection preferred
- Must live within 3 driving hours to Nashville or Knoxville, Tennessee
- Willingness and ability to obtain additional licensure as needed
- Ability to work overnight, 32 hours per week, including every other Saturday and Sunday, from 9:30 p.m. to 6:00 a.m.
