About the role
Key responsibilities & impact- Assess and evaluate members' needs and requirements in a telephonic environment
- Guide members and families toward appropriate care and wellbeing resources
- Manage members' physical, environmental, and psychosocial health issues
- Identify and resolve barriers to effective care
- Monitor patient care through assessments and evaluations to ensure progress toward desired outcomes
- Create member care plans as needed
- Perform health assessments over the phone
- Provide care planning and education on chronic conditions and gaps in healthcare plans
- Make independent decisions regarding work methods while following established guidelines and procedures
- Work with an assigned member population
Requirements
What you’ll need- Bi-Lingual (English/Spanish)
- Bachelor's degree
- 3 - 5 years of clinical acute care experience
- Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
- Passion for contributing to an organization focused on continuously improving consumer experiences
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
- Self-provided internet service with at least 25 Mbps download speed and 10 Mbps upload speed
- Availability for working hours between 8:00am-7:00pm Central Standard Time, with core hours primarily 8:00am-5:00pm CST
- Occasional travel to Humana offices for training or meetings may be required
