About the role
Key responsibilities & impact- Provide compassionate, patient-centered behavioral health support in a virtual setting
- Provide comprehensive biopsychosocial and standardized assessments for potential program enrollment
- Develop individualized support plans with patients and establish goals
- Provide community service resources and make referrals
- Provide enrollment and intake services
- Respond to telephone inquiries about mental health care and services
- Conduct safety assessments and planning by telephone or video
- Partner with patients to create sustainable, self-directed changes that promote health and well-being
- Collaborate with multidisciplinary team members to develop patient support goals
- Participate in multidisciplinary patient care conferences as needed
- Document interviews, consultations, referrals, contacts, and supportive services
- Complete documentation and reporting according to departmental standards and federal and state regulations
- Work in a constant state of alertness and safe manner
- Perform other duties as assigned
Requirements
What you’ll need- Must live in the four-state footprint: Missouri, Illinois, Wisconsin, or Oklahoma
- Master's degree in Behavioral Health or education equivalency for licensing
- Required professional license/certification appropriate to the state of work location: Illinois — LCPC, LCSW, or LMFT; Missouri — LCSW, LMFT, or LPC; Oklahoma — LADC, LBP, LCSW, LADC-MH, LMFT, LMSW, or LPC; Wisconsin — LCSW, LMFT, LPC, MFT-TL, APSW, or LPC-IT
- Experience with Epic
- Strong communication skills
- Teamwork skills
