About the role
Key responsibilities & impact- Evaluate and assess patients’ healthcare needs using established triage protocols and guidelines
- Participate in weekly huddles to review KPIs and ensure workflows meet expectations, including call-time limits
- Obtain provider input to determine appropriate clinical actions and recommendations
- Document symptoms, nursing assessments, advice, responses, plans, follow-up, and implementation in the EHR
- Follow policies, procedures, protocols, standing orders, and triage algorithms
- Coordinate and schedule same-day and future provider appointments
- Manage prescription refills per protocol
- Participate in CPC+ Care Coordination team processes
- Provide patient education on chronic disease, preventive care, and family health
- Participate in quality and performance improvement activities
- Forward pertinent medical information to appropriate departments for continuity of care

Requirements
What you’ll need- 3 years clinical experience with applicable patient care and administrative duties preferred
- Knowledge of disease processes, normal growth and development of all age groups required
- Nursing assessment skills with/without visual inspection utilizing basic nursing theory required
- Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required
- State of Colorado Registered Nurse or valid compact state license required
- Must be able to live and work primarily in one of the listed states: Arizona, Colorado, Connecticut, Florida, Georgia, Idaho, Illinois, Kansas, Massachusetts, Maryland, Michigan, North Carolina, New Jersey, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Virginia, Washington, or Wisconsin
