About the role

Key responsibilities & impact

Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery
Manages and directs RNs, LPN/LVNs, and Home Health Aides
Supports quality and appropriate home care services
Receives referrals, determines required services, and evaluates the Agency’s ability to meet client needs
Assigns appropriate clinicians to cases
Provides guidance and information to clinical staff
Ensures client needs are met according to care plans
Assists clinical staff with priorities, goals, and progress evaluation
Attends case conferences and clinical meetings
Participates in the clinical on-call rotation by providing telephone support
Completes client care visits or shifts as directed
Provides support and direction to Agency staff, healthcare professionals, clients, and families
Educates Agency staff on clinical services, policies, and procedures
Ensures accurate, complete, and timely clinical documentation
Completes quarterly or more frequent client chart reviews
Assists with screening, interviewing, orientation, and training of new personnel
Conducts annual performance evaluations for clinical staff
Performs client admissions, supervisory visits, and reassessments
Develops client care plans
Manages medical practitioner orders for client care and treatment
Performs other duties as assigned

Requirements

What you’ll need

Registered Nurse with a Bachelor of Science in Nursing (BSN) degree with two (2) years' public health nursing experience, or a Registered Nurse with three (3) years' public health nursing experience
Current license as a Registered Nurse in the state(s) of practice
A minimum of two (2) years of homecare experience is required
Supervisory experience is preferred
Excellent verbal and written communication skills and strong interpersonal skills
Knowledge of home health federal and state regulations
Knowledge of reimbursement sources and documentation requirements within home health care
Must be a licensed driver with automobile insured in accordance with Agency and state requirements
Current CPR certification
