About the role

Key responsibilities & impact
  • Identify, assess, plan, coordinate, implement, monitor, and evaluate options and services for individuals requiring medical and/or vocational case management
  • Collaborate and communicate with claimants, families, physicians, and other healthcare team members
  • Conduct on-site visits with claimants, medical care providers, and employers as needed
  • Evaluate treatment plans and assess injured workers, families, and providers
  • Visit injured workers' homes and families when needed to assess strengths, resources, problems, and psychosocial, financial, and medical history
  • Evaluate current status, diagnosis, prognosis, functional status, treatment plans, and needs
  • Develop physical, medical, and/or vocational service plans through interdisciplinary collaboration
  • Coordinate services and resources and negotiate or utilize approved vendors
  • Complete case management activities and prepare client reports using computer software and telephone
  • Provide vocational rehabilitation plans to facilitate return to suitable, gainful employment
  • Visit work sites when requested or required by the client/employer
  • Participate in marketing activities for a prescribed geographical area

Requirements

What you’ll need
  • Degree in nursing or other degree and appropriate experience
  • RN with minimum of 3 years direct patient care
  • Current RN licensure, if applicable
  • Case management/Vocational Rehab experience preferred
  • Current CCM or other applicable certification or eligible for certification
  • Strong clinical knowledge base
  • Excellent written and verbal communication skills
  • Strong analytical skills
  • Ability to work independently under general management direction
  • Ability to prioritize and organize tasks
  • Ability to travel long distances and walk distances to and from medical facilities
  • Ability to use computer software and telephone
  • Ability to meet Quality Management standards of BHN
