About the role
Key responsibilities & impact- Serve as a trusted frontline public health worker

Requirements
What you’ll need- High School Diploma or GED
- Experience in health care, public health, community-based services, human services, care coordination, behavioral health, or social services
- Knowledge of local community resources
- Experience with Medicaid, rural populations, behavioral health, chronic disease, maternal-child health, or underserved communities
- Completion of, or willingness to complete, a Michigan-recognized CHW training program
- Must live near Boyne, Charlevoix, or Harbor Springs areas
- Must live in the Central Region of Northern Michigan
- Must be vaccinated or have lab-confirmed immunity for Measles, Mumps, Rubella, and Varicella
- Must receive a flu vaccine during the flu season in the year hired and annually thereafter, or have an approved medical or religious exemption
